The NAYA Family Center, through a generous grant from the Spirit Mountain Community Fund, has remodeled a room specifically for use by our community elders.

REASSURANCE: Services include regular friendly telephone calls and/or visits to physically, geographically, or socially isolated individuals to determine if they are safe and well, if they require assistance, and to provide reassurance.
OAA Case Management: Service for individuals aged 60 and over or 56 years old with a disability, who need ongoing support to remain independent. The service aids in problem solving and connects the participant to resources and community support that will enhance independence.
Options Counseling: This is not traditional Case Management as described above but is a related service. Options Counseling is short-term facilitation that supports informed long-term care decision-making by the participant. The trained Options Counselor provides individualized person-centered decision support
to consumers, family members and/or significant others, regardless of income and meets with them in the home, office, or over the phone depending on the person’s preference.
Options Counselors work with individuals and/or families, to help them understand their strengths, needs, preferences and unique situations. The Options Counselor creates an action plan for the consumer to navigate possible support strategies, and tactics based on the choices available in the community. Options Counseling can be a stand-alone service for a participant or can lead to longer-term case management.
Focal Point Activities: These are the various NAYA Family Center activities, events and functions that are organized or coordinated by NAYA Family Center both on and offsite, that our NAYA Elders can attend and participate. This also includes activities that are planned for the Elders in the Elders room. The Elders Yoga class is part of our Elders focal point activities and is offered at the same time as our Tai Chi class on Tuesdays and Thursdays from 11am to 12pm Evidenced
Health Promotion: NAYA Family Center offers the Elders a Yoga and Tai Chi program that supports seniors to increase and retain balance to avoid falls. The instructor offers classes at NAYA Family Center in the Elders room on Tuesday and Thursday from 11am to 12pm.
Congregate meals: NAYA Family Center provides food and nutrition services that to our Elders who are 60 and over. The meals are twice daily Monday – Friday from 9:30am – 10:30am for the breakfast and 12:30pm – 1:30pm for the lunch. As part of the NAYA Family Center congregate meal program Nutrition classes and information are provided monthly, for participants, through our partnership with the Oregon State University Extension SNAP-Ed & Urban Native Indigenous Program.
The Elder program services are provided by NAYA Family as a culturally specific organization and through our Enhancing Equity contract with Multnomah County, which focuses on the needs of a particular cultural, racial and/or ethnic group in the entire County and provide services chosen from a menu of possibilities, based on community need and organizational capacity.
